Access to and Transfer of Medical Records<\/h2>\n

Our practice respects your right to access your health information. To maintain absolute data security, patient privacy, and compliance with Victorian health records legislation, we require a formal process to be followed when requesting access to or a transfer of your medical records.<\/p>\n\n

1. How to Request Access or a Transfer<\/h3>\n

Patients must provide a formal, written request to access their medical records or to request a transfer of their history to a newly nominated medical recipient. To safeguard your privacy, this request must contain the following information:<\/p>\n

  • Full name and current residential address<\/li>\n
  • Date of birth<\/li>\n
  • A copy of valid photo identification (such as a current driver licence or passport)<\/li>\n
  • The patient\u2019s signature and the date of the request<\/li>\n<\/ul>\n\n

    2. Secure Digital Delivery<\/h3>\n

    Once your written request and identity have been verified and processed by our team, we will generate a highly secure, encrypted link for you to safely download your digital records. This modern delivery method strictly minimises the risk of data breaches and ensures your sensitive information remains confidential.<\/p>\n\n

    3. Transferring to a New Doctor<\/h3>\n

    If you choose to transfer your ongoing care to another medical clinic, your new practitioner is entitled to a comprehensive treatment summary or a copy of your full records. Upon receiving a formal transfer request from your new clinic\u2014which must include your signed patient authorisation\u2014we will securely transmit your history directly to their practice.<\/p>\n\n

    4. Administration Fees<\/h3>\n

    While we endeavour to make this process as seamless as possible, a reasonable, cost-reflective administration fee may be charged to cover the administrative staff time, data preparation, and secure processing required to extract and provide copies of medical records. Our reception staff will advise you of any applicable fees prior to processing your request.<\/p>\n\n
