records or to request a transfer of their records to their nominated recipient.<br>Request should contain the following information:<\/p>\n<ul>\n<li>Name and address<\/li>\n<li>\nDate of birth<\/li>\n<li>\nIdentification, such as a driver licence<\/li>\n<li>\nPatient&#8217;s signature and date<\/li>\n<li>We will create a secure link for you to download<br><\/li>\n<\/ul>\n<ul>\n<\/ul>\n<p>\nIf a patient wishes to transfer to another doctor, the new practitioner is entitled to a treatment summary or a copy of the records.<br>A reasonable cost may be charged for providing copies of medical
