Patient’s should ideally provide a written request for access to their medical records or to request a transfer of their records to their nominated recipient.
Request should contain the following information:

  • Name and address
  • Date of birth
  • Identification, such as a drivers licence
  • Patient’s signature and date
  • Specify how you wish to have the information provided (fax, post, or email

If a patient wishes to transfer to another doctor, the new practitioner is entitled to a treatment summary or a copy of the records.
A reasonable cost may be charged for providing copies of medical records.
